Insurance Information

In-network insurances: Aetna, Meritain Health, United Healthcare, Blue Shield of California.

 

Medicare: Dr. Kristy is a non-participating physician. Payment will be collected at time of service. A claim will be submitted to Medicare on your behalf and reimbursement will be sent directly to you.

Out of network insurance: A claim can be submitted to your insurance on your behalf. If you choose to process your claim without paying at time of service, you will be responsible for any amount applied to your deductible and/or co-insurance when the claim is processes. If you pay the cash rate at time of service (listed below), your insurance will reimburse you directly based on the details of your plan.

Time of service discount/ cash rate: 

Initial visit: examination and treatment, if indicated: $170

Follow-up visits: $75 

 

Patients that have chiropractic benefits with their health insurance will continue to pay their specific plan co-insurance or copay amount. If more chiropractic treatments are needed after the upward limit has been exhausted, the amount collected for an office visit will be $75.

Financial Policy

New Element Chiropractic is happy to work with patients to provide the best quality of service. The office polices outlined are to ensure a positive experience as a patient of New Element Chiropractic. This financial policy is intended to inform you of our office billing procedures.

Re-schedule/ Cancellation policy:

New Element Chiropractic has a re-schedule and/or cancellation policy that patients must give notice 24 hours in advance of their appointment. If you cancel or request to reschedule within 24 hours before the appointment window, you will be charged a full office visit fee of $75.00. No-shows will be charged the full out-of-pocket fee for their appointment.

Insurance:

If you have health insurance that you believe has chiropractic benefits that would be covered in this office, we will verify your insurance coverage for you. Once your eligibility and coverage is determined, we will file all your insurance claims for you to the extent that your policy permits. Your health insurance company will then send you a statement (explanation of benefits) explaining what services were paid for on their behalf as well as any amount that is your responsibility (co-insurance or deductible).

 

It is the policy of this office that you pay your patient portion (copay or co-insurance) at the time of visit.

 

On occasion, the insurance company will send a check for payment of services to patient. Should that occur, please endorse the check and bring payment directly to New Element Chiropractic. If there is an overpayment from your health insurance, New Element Chiropractic is obligated to reimburse the insurance company directly and any overage will be remitted to them. 

 

Health insurance is an agreement between the patient and the insurance company. IT MUST BE UNDERSTOOD that this clinic DOES NOT promise than an insurance company will pay for services rendered. This clinic will not enter into a dispute with an insurance company for reimbursement.

 

Automotive Insurance:

If your visit is due to an automobile accident, we will work with your car insurance and file all claims for you. 

 

Non-Insured:

We request 100% of the each visit to be paid in full at the time of service. We offer affordable cash options to uninsured patients.
